This file is a note area of sorts for keeping track of "problems" that
need to be fixed.

Noted means the problem has been found to exist.
Diagnosing means that the problem is being investigated for a cause
Fixing means that a solution is being sought, or being worked on.

Once the problem goes away, it is removed from this file.

Workarounds should be noted with the problem.
